The UEFA Plaque merupakan sebuah penghargaan yang diberikan oleh UEFA kepada Juventus FC atas keberhasilan mereka menjadi juara di tiga kompetisi yang diadakan UEFA.[1] Juventus menjadi tim Eropa pertama yang pernah menjuarai tiga kompetisi UEFA (treble) yaitu Liga Champions, Piala Winners dan Piala UEFA pada 1985.[2]
